Integrating Systems
and Services for Children and Youth with Special Health Care Needs PA DOH Youth Parent Professional Forum NWD September 28, 2009 6:00-9:00 pm Youth and Family Forum
September 29, 2009 9:00 AM -12 Noon For All Interested
The Pennsylvania Department
of Health, along with its partners, the Pennsylvania Academy of Pediatrics, the
PEALCenter and the Pennsylvania Health Law
Project have received
a federal state implementation grant for Integrating Systems
and Services for Children and Youth with Special Health Care Needs. As
part of that grant, regionally based, parent/youth/professional forums are
being held across Pennsylvania. The over-arching goals of these
forums are to create sustainable improvements locally, regionally and
statewide, to systems serving children and youth with special health care needs.
In the Pennsylvania Department of Health, Northwest District, this means bringing
parents, youth, providers and systems people together, to discuss and
address common concerns for our region. Goal 1:Strengthen system capacity by integrating local, regional and statewide
parent/professional, community, providers and systems partnerships to advocate
for the needs of children and youth with special health care needs (CYSHCN).
Goal 2:Ensure all children in PA receive comprehensive, coordinated care in a
medical home by expanding provider capacity for CYSHCN, with an emphasis on the
underserved. Goal 3:Integrate the educational, medical wellness and community systems to
ensure that CYSHCN’s make successful transitions to all aspects of adult life. For more information see http://www.paaap.org/cyshcn.php
